Please answer questions to the best of your ability. We will not ask for any documentation, proof of income or NDIS information. Your information will be sent to our team for review.

Why do we collect the information requested in the form?

  • Some information is required for our reporting to the Department of Communities, which helps us to provide this Disability Legal Connect Platform
  • We ask what other people or businesses may be involved in your query or issue to help check conflicts of interest within our partner network of legal and advocacy service providers 
  • We may have team members with experience in certain areas of disability legal help that would be best placed to help you

 

Where does the information go when it is entered into the form?

  • The Disability Legal Connect project is connected to a group of partner service providers who require your information in order to help you with your question or issue

 

Can I request that my information is not shared with anyone except the one service provider who first receives the information?

  • Yes, you can request this within the form once you start completing your communication preferences.
  • You can find out more about how your information is stored, shared and used using the links to the different legal disclaimers and conditions in the footer of this DLC website.
